My hydra form ammys are as follows:
Strange (protect from exp drain)
Death Protection (60% resistance to death)
Eagle (gives flight, I want falcon here)
Greater Vitility (+25 HP) alternate Greater Protection (reduces all received damage)
Greater Vitality alternate Free Action (90% resist on paralyze and slow) second alt Greater Protection

I macro the use of all my alternates. Vitality ammys have the down side of subtracting directly from your HP when after shifting.

For example you have 100 hp in naga that would be 250 in hydra. Add 50 for 2 greater vitality makes 300 in hydra. You are at full hp and shift back to naga. Your hp drops to 120 then the amulets subtract and your down to 70. Just another one of those things. While I have yet to test this at low health it might possibly cause death. So I reccommend a certain bit of care when using Girdle of Constitution and Amulets of (Greater) Vitality.
